The process of transferring the management authority of Social Insurance agency when changing the business location

Answer:

After the company has changed its Certificate of Business Registration to change the registered office address, the company needs to follow the document procedures to transfer the place of participation in insurance to the Social Insurance Agency where the new registered office is located (According to Official Letter No. 1366/BHXH-THU Guidelines for the process of transferring participation in social insurance, health insurance, and unemployment insurance) as follows:

Step 1: Report the reduction of social insurance, health insurance, and unemployment insurance and close the social insurance account with the Social Insurance Agency of the place of departure

– Required documents:

– Receipt of suspension of social insurance participation (Form 106, 2 copies)

– Certified copy of the business license (1 copy)

– List of employees participating in social insurance and health insurance (Form D02-TS, 1 copy)

– Valid health insurance card (1 card/person)

– Proof of payment – if any (Copy)

Step 2: Report the increase of social insurance, health insurance, and unemployment insurance to the Social Insurance Agency of the place of arrival

– Required documents:

– The file sent by the Social Insurance Agency as mentioned above

– Mandatory receipt of social insurance registration (Form 101, 2 copies)

– Application for social insurance and health insurance participation (Form 01/ĐKBB, 1 copy)

– Decision on establishment or business/license (Certified copy)

– List of employees participating in social insurance and health insurance (Form D02-TS, 1 copy)

– Declaration of social insurance and health insurance participation for first-time participants (Form TK1-TS, 1 copy/person)

– Notice of social insurance and health insurance payment results from the previous participating agency (Form C12-TS, 1 copy)
